In accordance with the requirements of paragraph 5 of the above, I hereby certify that I am in
compliance with the prevailing wage and minimum wage provisions of the New York State
Labor Law, the Administrative Code of the City of New York, and all applicable rules and
regulations adopted pursuant thereto.
I understand that in the event of any breach or violation of the foregoing statutes, rules and
regulations, the parties responsible therefor shall be subject to the penalties and sanctions
provided by law.
