ATTACHMENT I – All Hazards Incident Management Teams

Adjunct Instructor Application
This attachment is to be completed for those interested in teaching, and have experience as a member of an All
Hazards Incident Management Team (IMT). Successful applicants will be able to instruct a variety of IMT, providing
they’ve completed the requisite training, and have responsibilities as Incident Commanders, Sections Chiefs or Unit
Leaders for the programs identified under the “Certificates of Completion” section below.
In addition to the requirements found on Page 1 of the Adjunct Instructor General Application, in order to be
considered for this track, applicants must provide:

Biography describing your background and experience as a member of an Incident Management Team. Bio
should include:
Incident Management Team responsibilities spanning the past 5 years during incidents that went
longer than one operational period, or involved written action plans and multiagency coordination.
Specific positions held during an incident.
Note: Only those currently serving on recognized Incident Management Teams will be considered for adjunct
instructor positions. All others may be used as secondary or unit instructors at DEMA’s discretion. Biography should
include a “Statement of Currency” detailing how your responsibilities provide you with the opportunity to remain
current as a member of an IMT.
Recommendation from your employer or IMT Commander, vouching for your participation in the above
incident, and your overall experience as a member of an IMT.
Certificates of Completion for the following programs:
G-300 – Intermediate Incident Command (required for all programs)
G-400 – Advanced Incident Command (required for Incident Command and Section Chief programs)
Certificate of Completion for each Position Specific IMT course applying for
Train-the-Trainer (TtT) Certificate of Completion for each Position Specific IMT course applying for
Additional Documentation Required for all IMT Applicants
Include an Incident Action Plan or other documentation (ICS Form 201) showing your IMT position on an
incident or event that went beyond one operational period (for each position applying for).
A signed copy of the verification page of the Position Specific IMT task book for each position applying for
DEMA may provide train-the-trainer opportunities if applicant is otherwise qualified and eligible for this track.
Note: DEMA Training Branch Manager and the FEMA All Hazards IMT Program Manager shall reserve the right to be
the final authority of any individual’s qualification or abilities, and may waive experience and qualifications as
warranted.
Certificates must be issued by the Federal Emergency Management Agency/Emergency Management Institute,
Center for Domestic Preparedness, Texas Engineer Extension Service, National Wildfire Coordinating Group, or the
Arizona Department of Emergency and Military Affairs.
