This attachment is to be completed for those that have experience in Hazardous Materials Response. Depending
on level of experience, successful applicants will be able to instruct one or more of the following classes. Instructors
are not required to teach all modules of the Hazardous Materials Technician course.
MAU-100 Hazardous Materials Response Awareness (8-Hour)
MAU-200 Hazardous Materials Response Operations (24-Hour)
MAU-201 Hazardous Materials Response Operations Refresher (8-Hour)
MAU-300 Hazardous Materials Response Technician (200-Hour) selected modules/topics
MAU-301 Hazardous Materials Response Technician Refresher (24 to 40-Hour)
In addition to the requirements found on Page 1 of the Adjunct Instructor General Application, in order to be
considered to instruct one or more classes in this track, applicants must provide:

Recommendation from your employer, vouching for your overall experience and participation in recent
hazardous materials incidents and programs.
To Instruct MAU-100 (Awareness), MAU-200 (Operations), or MAU-201 (Operations Refresher):
DEMA MAU200 Certificate of Completion or equivalent 24-hour Operations-level course
Biography of HazMat response experience spanning the past 3 years at the Operations level.
Certificates of Completion showing 240 hours of Hazardous Materials related training you’ve received
(attach certificates listing hours) training should encompass the past 10 years.
Proof of completion of an adult teaching methodology course, or other experience as an adult educator
Include an Incident Action Plan or other documentation (ICS Form 201) detailing a HazMat incident or
incidents you were involved with at the respective level (Operations/Technician).
To Instruct MAU-300 (Technician), MAU-301 (Technician Refresher):
All requirements above, plus;
DEMA MAU300 Certificate of Completion or equivalent 200-hour Technician-level course
Biography of experience detailing your areas of expertise, along with the following:
3 years continuous assignment on a Hazardous Materials Response team at the Technician-level.
3 years recent experience as a Hazardous Materials instructor (DEMA, NFPA, IAFF, IFSTA
AZCFSE)
Proof of completion of an adult teaching methodology course, or other experience as an adult educator
Letter from a sponsoring agency (authority having jurisdiction) indicating compliance with OSHA 29 CFR
1910.120 q6. for continuing education (technician level).
Include an Incident Action Plan or other documentation (ICS Form 201) detailing a HazMat incident or
incidents you were involved with at the respective level (Operations/Technician).
Note: Only those currently serving in emergency response/emergency services disciplines with Hazardous
Materials Response responsibilities will be considered for lead instructor positions. All others may be used as
secondary or unit instructors at DEMAs discretion. Biography should include a “Statement of Currency” detailing
how your responsibilities provide you with the opportunity to remain current HazMat Response at either the
Operations or Technician level.
Additional Documentation Required for all HazMat Programs
Include an Incident Action Plan or other documentation (ICS Form 201) detailing a HazMat incident or
incidents you were involved with at the respective level (Operations/Technician).
Instructors for the HazMat Technician class are not required to teach all modules. Please select your areas of
expertise from the options below, and whether you feel you’re qualified to serve as Lead or Support instructor:

DEMA may send applicants to a Train-the-Trainer program if otherwise qualified and eligible for this track.
Note: DEMA Training Branch Manager shall reserve the right to be the final authority of any individual’s qualification
or abilities as Lead or Secondary instructors, and may waive experience and qualifications as warranted.
Certificates for HazMat Awareness, Operations and Technician training must be issued by the Federal Emergency
Management Agency/Emergency Management Institute, Center for Domestic Preparedness, Texas Engineering
Extension Service, National Wildfire Coordinating Group, Phoenix Fire Department, the Arizona Center for Fire
Service Excellence, or the Arizona Department of Emergency and Military Affairs.
Certificates counted toward number of hours of training received must be from recognized Hazardous Materials
training or conference providers.
