ASSUMPTION OF RISK/STUDENT CONDUCT AGREEMENT FOR
PARTICIPATION IN COCC SPORTS/INTRAMURALS
Many Club Sports/Intramural activities involve risks of bodily injury, property damage, and other dangers associated with participation in such activities.
Dangers peculiar to activities normally engaged in include, but are not limited to: Broken bones, cuts-scrapes-sprains, bruises, concussion, heart attack.
Each participant in Club Sports/Intramural activities should realize that there are risks, hazards, and dangers inherent in such activities and in the training,
preparation for and travel to and from activities.
Each participant in Club Sports/Intramural activities must be covered by an accident insurance policy.. It is the student's responsibility to secure an
accident/injury insurance policy. Central Oregon Community College does not provide primary insurance for Club Sports/Intramural participants. COCC
provides catastrophic insurance with an extremely high deductible. The participant is responsible for meeting the deductible.
It is the responsibility of each participant to participate only in those activities for which he/she has the prerequisite skills, qualifications, preparations, and
training.
Therefore, in consideration of the benefits received from the Club Sports/Intramural activities he undersigned hereby assumes all risks of damages or injury,
including injury which may prove fatal, that may be sustained by him/her while participating in Club Sport/Intramural programs and activities or in the travel to
or from such activities.
The undersigned applicant thereby releases Central Oregon Community College, its faculty and agents from all liability of any nature for loss or damage to
personal property. The undersigned applicant further releases Central Oregon Community College, its faculty and agents from all liability for personal injury
resulting from the failure of the undersigned applicant or other students in the club to obey safety regulations and directions of the group leader, or resulting
from the exercise of judgment by the group leader in good faith in response to emergencies and exigencies which occur on the trip; provided however, that
nothing contained herein shall excuse the person assigned to be a trip leader by a member of the faculty from the responsibility to act with reasonable care
for the safety of the undersigned applicant during the course of the trip appropriate to the circumstances.
It is the intention of the undersigned that this agreement will be governed by the laws of the State of Oregon.
STANDARDS OF CONDUCT:
Students enrolled in an institution of higher learning assume an obligation to conduct themselves in a manner compatible with the college/university’s function
as an educational institution. When team members participate in an event sponsored or endorsed by COCC, the participant indicates, by their registration in
the event, they agree to represent their team and college/university in a way that does not detract from the reputation of the institution they are representing
and to act morally and display sportsmanship-like behavior, before, during and after any competition. Guidelines of conduct include, but are not limited to the
examples outlined:
Participants shall not:
Use drugs, except for medical purposes, while traveling, competing, socializing and or using facilities including lodging places.
Consume alcohol while traveling, competing or spectating.
Strike, attempt to strike or otherwise physically abuse an official, opposing player, spectator or coach.
Intentionally engage in or incite participants and/or spectators to engage in abusive or violent action.
Use obscene gestures, profanity or disrespectful language.
Violate any host college/university rules or policies.
Smoke while traveling, competing or using facilities at COCC
Violation of Standards of Conduct may result in:
Suspension from the game/match;
Suspension from the tournament;
Forfeiture of individual or team awards;
Ejection from the tournament hotel(s);
Forfeiture of all games/matches won and possible team elimination from current and future COCC sponsored or endorsed events.
