Your application has been selected for review in a process called “verification.” The law requires specific
information from your Free Application for Federal Student Aid (FAFSA) be compared with the actual information
of you and your family. If there are differences, your FAFSA information may need to be corrected. You must
complete and sign, and attach the appropriate documentation listed below and submit it to the Financial Aid Office
at the college.

Provide one of the following documents that indicate the student’s high school completion status when
the student will begin college in 2014-2015:
A copy of the student’s high school diploma.
A copy of the student’s final official high school transcript that shows the date when the diploma
A copy of the student’s General Education Development (GED) certificate or GED transcript.
An academic transcript that indicates the student successfully completed at least a two-year
program that is acceptable for full credit toward a bachelor’s degree. (Official college transcripts
must be evaluated by the Credential Analyst in the Enrollment Service Office.)
If you are a homeschooled student you must have your credentials verified by the Enrollment
Service Office at Prairie State College to obtain credit for successful completion of a secondary
school education in a homeschool setting.
If the student is unable to obtain the documentation listed above, he or she must contact the financial
