Complete the COLLEGE ASSESSMENT TESTS if you haven’t done so already. The test is
not timed and can be enlarged on the screen, if needed. Go to the Assessment Center, located in
Career Services in the Administration Building or see their website:

Complete the college ORIENTATION if you are new to college. Sign up by going to the
Counseling Department in the Administration Building or their website:

Complete the attached NEW STUDENT SELF-REPORT QUESTIONNAIRE.
Obtain DISABILITY DOCUMENTATION. (See reverse side for appropriate documentation.)
Bring your NEW STUDENT SELF-REPORT QUESTIONNAIRE and DISABILITY
DOCUMENTATION to one of the following new student walk-in times in the DSP&S

Students should check in half hour before each scheduled walk-in as students will be seen on a
FIRST-COME, FIRST-SERVED BASIS until full. There is no guarantee you will be seen on the day
you come.
PARENTS/GUARDIANS are welcome to attend DSPS understands that students often
benefit from a parent’s support as they are starting college.
Please make OTHER ARRANGEMENTS for CHILDREN UNDER 16. This is a business
appointment and children disrupt the process.

WHAT IS DISABILITY DOCUMENTATION?
In order to support your request for accommodations related to a disability, you must provide
DSPS with some documentation of that disability. Here are some types of documentation we
accept:
Your most recent IEP and Psycho-Educational Report. You might have it at home or you
can get it from your high school district student records office, if you received special
services in high school;
A report from your medical doctor or an agency that has your records, such as
Department of Rehabilitation or Veterans Affairs;
A report or from your psychologist/therapist/psychiatrist or other professional;
A report from your eye doctor or other medical specialist;
An audiogram;
DISABILITY DOCUMENTATION FORM - if you don’t have any of the above
documentation, you can take the Disability Documentation Form (attached) to your
doctor or other professional to fill out.
