This is a critical section. This is where you will not only list what your event is about and the
intended audience, but also where you will (At the end or where appropriate), indicate any budgets
being used, linkage to college goals, impact, etc. Alternatively, you can email a separate document
indicating the in depth look and financials surrounding this event, including if you are selling tickets
or expect revenue.

Fundraising Event (list organization)
If your event involves fundraising, please indicate the budgets involved and submit supporting
documentation detailing the fundraiser. Attach any communication with the Foundation or others.

Parking Lot Requested If you will be requesting ANY designated parking in a specific lot, please list the lot preferred.

Pre-Approved Coordinators ONLY
This section are for individuals who have been pre-approved to submit their own Facilities Work
Order request, AV Requests, Parking Requests, Calendar submissions, etc. and are largely for the
major, recurring events. If you are not sure if you are a Pre-Approved Coordinator, please contact
the VPA office.
If you are pre-approved: the VPA office will NOT submit these requests for you, but will provide
support and assistance as needed, and must be included on all correspondence for that reason.

What is a major event?
A major event is any event with an expected attendance of 50 people or more, or is at all open to
the public

President Cabinet minutes will reflect approval. You will be notified by the VPA Office, and if
approved will be assigned an event number.

What's the event number?
This is an auto generated number that MUST be attached to any/all requests related to the event.
This is an automatic indicator to Facilities, AV, the Calendar, etc. the event has been vetted and
approved.

I am a club/club advisor and go through
Student Affairs - how does this form
affect me? What happens if I do/do not
need a space in I400?
Follow the same procedure you normally would. Once your Club Advisor and the Dean of Student
Affairs approves the event, you MUST send the form to the VPA office to reserve ANY space outside
the I400 Building. Once reserved, your event/meeting will be issued an event number. If you are
meeting in the I400 Building, and your event is NOT a major event, you do not not need to notify
the VPA office.

I am a committee and just need a space
me?
Please contact the VPA office to reserve ANY space outside your department's building. If you are
meeting outside your department's building, submit this form for approval by your dean/chair.
Once approved, contact the VPA office to reserve the room. Once reserved, your event/meeting
will be issued an event number.

How far in advance to you need this
form?
MINIMUM OF 3 WEEKS FOR ALL RESERVATIONS AND CALENDAR SUBMISSIONS. For the larger
events, please plan accordingly. Events that are requested under three weeks may be subject to
additional approvals and may not be accommodated.
