If you have received a traffic citation or a citation for any infraction you may submit this form to request
adjudication of your case without an appearance at the El Dorado Superior Court (Court). Submit the
completed form and proof of correction (when applicable) to the location listed on your citation:

Note: Requests must be submitted no later than SEVEN (7) DAYS PRIOR to your appearance. Upon receipt
of a complete form, your appearance will be vacated, and the Court order will be issued within 10 court days.

imposed will be due within 30 days from the Court order.
2b. Payment plan with traffic school. I understand that full payment of the amount imposed will be due
within 90 days from the Court order and an additional $60.00 administration fee will be added to the fine
balance. Partial or full payments will be accepted.
2c. Full payment of amount imposed without traffic school. I understand that full payment of the
amount imposed will be due within 30 days from the Court order.
2d. Payment plan without traffic school. I request the following monthly payment: $50.00 $25.00
