RELEASE AND WAIVER OF LIABILITY
I, _____________________________________________, a student enrolled in the Criminal
Justice program at Weber State University, desire to participate in an INTERNSHIP PROGRAM
or FIELD EXPERIENCE. In consideration of the work, study, observation, and experience
furnished to me, I do hereby expressly waive, remiss and forever discharge and release the State
of Utah, Weber State University, WSU Department of Criminal Justice and its employees,
agents, and volunteers, the State, County, or Municipal agency to which I am assigned, all of
their officers, agents and employees, from any and all claims of any kind, nature, or character,
whatsoever, by reason of any act, or acts, of commission or omission, of the State of Utah,
Weber State University, WSU Department of Criminal Justice Program, the State, County, of
Municipal Agency to which I am assigned, all of their officers, agents and employees, in the
operation of the INTERNSHIP PROGRAM or FIELD EXPERIENCE. Claims based upon the
sole negligence of the State of Utah, Weber State University, WSU Department of Criminal
Justice Program, the State, County, or Municipal Agency, are expressly not waived.
This release is expressly intended to cover and include all claims, several or otherwise,
present or future, which can or may ever be asserted as a result of injuries or damages sustained
by me while a student participating in the INTERNSHIP PROGRAM or FIELD EXPERIENCE.
This release is entered into voluntarily and with the understanding that such an internship of field
experience is not required for a major in the Criminal Justice Program, I expressly assume the
risk of injuries which may occur as a result of my participation in this program as I am aware of
the potential dangers involved. This release entered into voluntarily with a full understanding
of its meaning and legal effect.
