The Conservator's Account has been presented for allowance and certified under the penalties of perjury by the
accountant.
The required notice has been given in hand to the Protected Person.
Any required notice has been given to or waived by all interested persons; or their assents are on file with this Court.
No objections were made or any objection has been withdrawn or a hearing held thereon.
Any Guardian ad litem appointed has filed a report and/or assented to the Account; or the report of the Guardian ad
litem has been considered by the Court.

THE COURT DECREES AND ORDERS:
If this is the Final Account and it is allowed, the distributions as stated therein are also allowed as the final and
complete settlement of the estate. The Conservator is discharged from this trust and office. The Conservator and any
surety on the bond are released and discharged from further claim or demand of any interested person.
