apprenticeship under the supervision of the above-named judge/attorney, who practices in Vermont and has been
admitted to practice before the Vermont Supreme Court for not less than three years prior to the commencement of the
apprenticeship. I further certify that I have carefully arranged with that judge/attorney a systematic course of study to
prepare me for the general practice of law, including, but not limited to, the subjects tested on the Uniform Bar
Examination and related Vermont law. I understand that, pursuant to Rule 7(d) of the Rules of Admission, this
apprenticeship shall consist of not less than 25 hours of study within 7 consecutive days or not less than 30 hours of study
during a period of 14 consecutive days.

pursuing the Law Office Study Program, you must send this completed form to: Board of Bar Examiners, 32 Cherry
Street, Suite 213, Burlington, VT 05401. A registrant's failure to timely file a Notice of Commencement will result in
the withholding of credit for study, except for extraordinary instances. You are advised to retain a copy of this
document and read the Rules of Admission to the Bar of the Vermont Supreme Court at
https://www.vermontjudiciary.org/sites/default/files/documents/900-00014_0.pdf. Note: If any part of the
apprenticeship is to be completed outside Vermont, you must seek approval from the Board of Bar Examiners.
