Sign up for a jury time at the music office BY NOON WEDNESDAY BEFORE FINALS.
Pay any fees for accompanying used before juries BY MONDAY OF THE LAST WEEK OF CLASSES.
You will not be able to sign up for juries until your accompanist fees are paid.
Make sure that this form and the “Jury Adjudication Form” are printed BACK TO BACK and both
sides of form are completely filled out, preferably typewritten.
Copies are NOT to be made in the music department office; use the library or other means to copy.
