VERY IMPORTANT NOTES FOR GUARDIANS:
This form may ONLY be used for a change of address of ward within Pinellas County.
If you are relocating the Ward from Pinellas to Pasco County, contact your attorney as this
requires a notice of the change filed with the court which states the compelling reasons for
relocation of the ward and how long the guardian expects the ward to remain in Pasco, per FS
744.1098(2).
If you are relocating the Ward to anywhere other than Pinellas or Pasco Counties, contact your
attorney as this requires a petition for an order authorizing the removal of the ward and shall
include in the petition the compelling reasons for relocation and longevity of such per FS
744.1098(1). The guardian shall also file a petition for a change of venue per FS 744.1097(3) and
Rule 5.050(b).
