We agree on the following issues:
We agree to attend voluntary mediation. We understand that if we reach an agreement in mediation, our agreement
will become an order of the court. We understand that if we do not reach a full agreement and one parent/party
decides to file a motion within six months of our mediation date, this mediation date will satisfy the requirement that
we attend mediation before our court hearing. We understand that this mediation is confidential and will be limited to
one session. We certify that there was a prior order filed and that we have not been to court or mediation regarding
visitation/custody of our children for the past two year.
