Your 2019-2020 Free Application for Federal Student Aid (FAFSA) was selected by the Department of
Education for review in a process called verification. Federal law requires OLLU to verify the information you
and your parents reported on your FAFSA before awarding you federal aid. If there are differences between
your FAFSA information and your financial documents, we will submit corrections electronically. You must
complete this verification form, sign and date it, and submit it to the financial aid office as soon as possible.
This will ensure that your financial aid will not be delayed.

• Yourself and your parent(s) including stepparent even if you don’t live with your parents, and
• Your parents’ other children even if they don’t live with parent(s), if (a) your parents will provide more
than half of their support from July 1, 2019 through June 30, 2020, or (b) the children would be
required to provide parental information when applying for Federal Student Aid, and
