DECLARATION OF ADDITONAL FUNDING FORM
INTERNAL USE – OVC GRADUATE PROGRAMS AND RESEARCH
OVC graduate students who have received an OVC Graduate Student Stipend (i.e. Scholarship, Fellowship or Entrance
Award) are required to submit a Declaration of Additional Funding Form to ovcsas@uoguelph.ca every semester to
initiate payment for the following semester. Please complete this form electronically. Failure to submit this form may
result in a delay of payment. For details, refer to OVC Graduate Stipend Support, Terms and Conditions.
Submission Deadline Windows by Semester
Summer: April 1-15 *May 1
Fall: August 1–15 *September 1
Winter: December 1-15 *January 1
*Amendment Required - If you receive additional funding from the time you submit the Declaration of Additional
Funding Form

first two weeks of the semester; failure to do so may result in
a delay of stipend payment and/or retro-active recovery
