The Ho-Chunk Nation is the “payer of last resort”. Therefore, applicants must seek assistance from other
funding sources, such as their local Johnson O’Malley Program, or program scholarships, prior to
applying for the COVID-19 Response Relief Printer program.
COVID-19 Relief Response Printer Purchase
Description: Due to the COVID-19 pandemic and remote learning requirement, assistance is
available to purchase a printer and additional ink cartridges for students in grades K-12
attending public or private/parochial school. The printer purchased will be the HP Deskjet 3755
Inkjet Multifunction Printer Color Copier/Printer/Scanner with additional black and color ink
cartridges. If students have received a printer during this fiscal year (July 1, 2019-April 28,
2020), they are not eligible to receive a printer, but as of April 28, 2020, can be reimbursed for
additional ink (1 color cartridge, 1 black cartridge) they purchase through June 30, 2020. There
will be no reimbursement available for printers and ink cartridges that have been purchased
before April 28, 2020.
Required Information (check off as each is obtained):
Completed COVID-19 Printer Program Application
Receipt for Ink purchase (dated after April 28, 2020)
An end of school year 2020 report card will be due to the Education Department by June
30, 2020. Future computer requests will be denied until that report card has been
submitted.
All applications must be submitted to: SCR@ho-chunk.com, or they will not be
processed.
The deadline to submit applications for a printer is June 15, 2020.
