5. Paternity. Paternity is disputed and/or needs to be established for the children indicated

No father is named on the birth certificate. I believe the father is (name of who

DNA Test Requested. The judge should order a DNA test to determine
if this is the biological father.
DNA Test Completed. The parties already took a DNA test which
shows the man above is the biological father; a copy is attached.
Parents Lived Together. The parties lived together at least 6 months

Admission. The man named above openly holds out the child as his
own and has accepted the child into his home.

The wrong man is named as the father on the birth certificate. The birth
