Authorized Person/Company listed below.

I (SRP’s Customer of Record) authorize SRP to release interval data to the authorized person or
company listed on this form. I understand that a separate form will be needed for each SRP service
address, as well as for each individual or company authorized. I also understand that the release of
interval data by SRP is not an authorization to make any changes to the customers’ electric account.
