This application should be filled out by students who wish to participate in the 2011 graduation
ceremony with coursework remaining after spring semester or have missed the spring application
deadline.
Please complete each section of this petition as indicated. You must be registered for summer
classes (no more than 2 classes) before this petition will be accepted. Your graduation date will be
Summer 2011.
1. Fill out this form and attach to an Application for Graduation *
2. Attach a print-out of your summer registration statement
3. All forms can be submitted between April 11-April 25, 2011
* If you have already applied for graduation for Spring 2011 term and it was denied or you did
not complete requirements, you will need to include a new Application for Graduation with
this petition.
Students who missed the Spring 2011 application deadline (March 1
st
) may also fill out this form in
order to participate in the 2011 graduation ceremony. Rather than listing courses, indicate
“complete Spring 2011” and sign at the bottom. Your graduation date will be Summer 2011. This
form can be submitted any time, but no later than April 25, 2011.

I understand that I must be registered in summer classes before this form will be considered. If I am
approved to participate in the graduation ceremony, I will complete my summer courses in order to
obtain my degree or certificate. If I do not complete my courses, I will reapply for graduation in the
semester I complete all requirements.

All my coursework will be completed by Spring 2011.
