hereafter referred to as “the DMV”:
You are ordered to set aside and revoke the DMV’s suspension of the petitioner’s driving
privilege commencing (date) through (date) or,
in the alternative, to show cause why you have not done so and appear for hearing on a
Writ of Mandate on (date) at (time) or as soon
thereafter as the matter may be heard in Department of the above entitled Court.
Petitioner is ordered to serve on respondent written notice of the time and place of the
hearing with a copy of the petition, the papers on which the petition is based and this Order
at least 16 court days prior to the date of the hearing if service is personal, and an
additional 5 calendar days if service is by mail.
