2020-2021 Verification Worksheet – Independent Student
A. INDEPENDENT STUDENT INFORMATION – Please fill in with the student’s information.

B. Independent Student’s Family Information – List the people in your household.
Include:
• Yourself.
• Your spouse, if you are married.
• Your children, if any, if you will provide more than half of their support from July 1, 2020 through June 30,
2021, or if the child would be required to provide your information if they were completing a FAFSA for
2020-2021. Include children who meet either of these standards, even if they do not live with you.
• Other people if they now live with you and you provide more than half of their support and will continue to
provide more than half of their support from July 1, 2020 through June 30, 2021.
Also, write in the name of the college for any noted above, who will be attending college at least half-time between July
1, 2020 and June 30, 2021, and will be enrolled in a degree, diploma, or certificate program.

C. TAX RETURN FILERS
If student and/or parent filed a tax return and did not utilize the IRS Data Retrieval process, a copy of the 2018 FEDERAL TAX
RETURN TRANSCRIPT(S) or SIGNED 2018 TAX RETURN must be submitted. To request a copy of the 2018 FEDERAL TAX
RETURN TRANSCRIPT from the Internal Revenue Service, call (800) 908-9946 or go to www.IRS.gov.
Important Note: If the student or spouse filed an amended 2018 IRS tax return, please contact your financial aid administrator
before completing this section.
Your 2020-2021 Free Application for Federal Student Aid (FAFSA) was selected for verification. Verification is the process used to confirm that
the data reported on your FAFSA form is accurate. The law states that the verification process must be completed before disbursing Federal
Student Aid. We may ask for additional information, if necessary, for clarification. If you have questions about verification, contact the
financial aid office as soon as possible so that your financial aid will not be delayed.
