Status 1: Initial College Enrollment – a first-time student who enrolls into a post-secondary educational
institution after high school graduation, or shows completion from an approved home school program.
Status 2: Transfer Student – Students transferring from one institution to a participating institution, who may
be eligible to receive a LIFE Scholarship, must submit an official transcript from the transferring institution.
Students who received the scholarship and transferred mid-year to this institution may be eligible to receive
the scholarship for the spring term. The student must submit an official document from the transferring
institution to verify that he/she met the requirements for continued eligibility at the beginning of the academic
year prior to the transfer. The transcript(s) must provide evidence of current credit hours earned and
cumulative grade point average.
Status 3: Continued Eligibility – a student who has completed a minimum of 30 credit hours of 101 level or
above by the end of the academic year/15 credit hours for mid-year students.
Status 4: Seeking Eligibility – a student who is currently not eligible; who has completed 30 credit hours or
15 credit hours if beginning mid-year at the 101 level or above; who has earned at least a 3.0 GPA; and who
would like the Student Financial Services Office to review their academic history for possible LIFE eligibility.
The LIFE Scholarship regulations require every recipient to certify he/she has not been legally judged to be a
delinquent, been convicted of or pled guilty to any felonies or any second or subsequent alcohol/drug related
offenses, within the past academic year, under the laws of South Carolina, or any other state or under the
laws of the United States. The legislation also states that a student in default on a federal or state student
loan or who owes a refund of federal or state grant is not eligible.
Any false information provided by the student or any attempt to expend any scholarship funds for unlawful
purposes or any purpose other than in payment or reimbursement for the cost of attendance at the institution
authorized to award the scholarship will be cause for immediate cancellation. Any student who has obtained
a scholarship through means of a willfully false statement or failure to reveal any material fact, condition, or
circumstances affecting eligibility will be subject to applicable civil or criminal penalties, including retroactive
loss of the scholarship. I further understand that I agree to the terms and conditions as outlined in the policies
and procedures for disbursing Title IV aid. I also affirm that I am presently not in default on any federal or
state student loans nor do I owe any refunds to any federal or state financial aid programs.
Your award may be adjusted without prior notice if new information is discovered. New information that may
cause your award to be adjusted includes, but is not limited to, conflicting documentation, failure to maintain
satisfactory academic progress, change in your enrollment status, and GPA. You will be notified if your award
changes. You may be held personally responsible for any liability that may result from the change. You may
also check your award online at MyMTC.MIDLANDSTECH.EDU. We recommend you check your award and
balance due prior to the fee payment deadline on your schedule.
I have not been convicted of any felonies. Further, I certify that I have not been convicted of an alcohol or
drug related misdemeanor offense during the preceding calendar year defined as 12 months from the date
of the start of school for the period of this award. If I am adjudicated delinquent, convicted or pled guilty
or nolo contendere to any felonies or any second or subsequent alcohol/drug related offenses under the
laws of this or any other state or under the laws of the United States, I agree to notify the Student Financial
Services Office by the start of school. I hereby give permission for a background check to be conducted to
verify the above. I understand additional information may be requested after the background check has been
conducted.
If you have previously attended a college or university, all official transcripts must be submitted to the
Admissions office. Eligibility will be based on a LIFE GPA which will be calculated from all grades and credits
earned at any eligible institution (in-state and out-of-state), including courses that do not transfer based on
the institution’s policy and college courses taken while in high school. I understand that non-degree credit
hours will not be used in calculating the “LIFE GPA” or credit hour requirement if I am enrolled in an Associate
Degree or higher.
