LIFE SCHOLARSHIP EARLY
GRADUATION APPLICATION
DEADLINE: MARCH 5, 2021
Students who complete all requirements for high school graduation prior to the official graduation date in May/June may be eligible to receive
the LIFE Scholarship if they meet all initial and general eligibility criteria. Please refer to the Scholarship guidelines posted on the Commission on
Higher Education’s (CHE) website at www.che.sc.gov.
EARLY GRADUATION DOCUMENTS
In order to be considered for LIFE Scholarship for Early Graduation, the following documents must be submitted to Midlands Technical College by
the deadline date:
1. Original and completed Early Graduation Application signed by student – submit to the Student Financial Services Office
2. Original principal letter (on school letterhead) indicating student has completed all high school graduation requirements by
the established December/January deadline – submit to the MTC Admissions Office
3. Original high school transcript(s) must be in an officially sealed envelope. Transcript(s) must include all grades through
December/January and include a cumulative GPA based on the SC Uniform Grading Policy/GPA Calculation Date–submit to the MTC
Admissions Office
4. Admissions Office Early Graduation/LIFE Agreement Form – submit to the MTC Admissions Office
5. All documents must be received by the Student Financial Services/Admissions Offices by close of business on March 5, 2021.
IT IS THE STUDENT’S RESPONSIBILITY TO ENSURE THAT ALL DOCUMENTS ARE RECEIVED BY MARCH 5, 2021.

Services and Admissions Office by the established deadline for a complete application and that all submitted information is true and accurate. I
understand that institution staff is not responsible for ensuring that my Early Graduation Application is complete for review and that institution
staff will not contact me, my parents/guardians or the institution concerning documentation that is missing and/or incorrect in my application. I
also understand that the final decision regarding my application will be based only on the documentation/information received by the deadline.
