EPIC@JU.EDU before 5:00 pm local time on or before
Monday, October 15, 2018, for projects starting January 7, 2019.

approval from the Institutional Review Board
or the Institutional Animal Care and Use
Committee prior to beginning any work on the
project. Funding will not be released until
IRB/IACUC approval is confirmed.
Applicants are invited to use the electronic signing feature built into this form. Simply click in the

the next person to repeat the process. Once all electronic signatures are obtained and returned to
the applicants, the applicants should save the file and include it with their submission email. Hand-
written signatures may also be obtained, scanned, and attached to the submission packet, but please
note, once a form is hand-signed and scanned in as a PDF, the electronic signature feature will be
de-activated and all other signers will have to sign with a hand-written signature, versus electronic.
