Teen Court Volunteer Agreement / Release
By signature below, I affirm, as do my parents/guardians that I understand and agree to the
following
conditions and responsibilities associated with participation in Teen Court of Clay County,
Florida, Inc.
•
Volunteers must be compatible with Teen Court’s purpose of promoting law-abiding behavior
•
Volunteers must be respectful to all participants throughout their service
•
Volunteers must abide by the Teen Court Dress Code
•
NO CELL PHONES, ELECTRONIC EQUIPMENT, HATS, GUM, OR SUNGLASSES
Dress Code
:
Compliance with the Teen Court Dress Code is mandatory.
No exceptions permitted or tolerated. Your attire
should reflect conservative and professional clothing. It is important to be professional with your actions and your
dress.
Male Volunteers: Must wear dress slacks, khakis, polo-type shirt or button-down dress shirt, (tie / jacket
is
encouraged). Shirts must be tucked into slacks and belted at the waist.
• No Denim Jeans or Shorts
• No baggy or athletic type clothing
• No Facial Piercings.
Female Volunteers: Must wear dress slacks, Skirts/dresses, blouses (with sleeves).
• No Denim Jeans or Shorts
• No Capri Pants
• No Leggings
• No Flip Flops
• No facial piercings
The right to participate in Teen Court may be revoked at the sole discretion of the Judge and/or Executive Director
for violation of the philosophy, purpose and rules of Teen Court of Clay County, Florida, Inc. As a volunteer in
Teen Court you are agreeing to the possibility of random drug testing.
It is further agreed the undersigned do hereby compromise and forever release, acquit, discharge, indemnify
and covenant to hold harmless, Teen Court of Clay County, Florida, Inc., its employees and Board of Directors,
City of Green Cove Springs, and Clay County its employees and officials, from all causes of action, claims
liability, demands, damages, costs, loss of service, medical expense and compensation, on account of or in any
way growing out of any and all known and unknown personal injuries, property damage, or any other type of
damage, which the undersigned may hereinafter have, individually and/or as parent/guardian or custodian of
said minor, resulting or growing out of the participation of the student volunteer in the Teen Court of Clay
County, Florida, Inc.
