Pursuant to section 59-101-430 of the South Carolina Code of Laws, as amended in 2008, to
attend a public university in the state a student must be a citizen or national of the United States or
an alien lawfully present in the United States. Winthrop University is required to verify the status
of all students prior to enrolling at the University.
Students must provide proof of citizenship or lawful presence in the United States to be eligible to
enroll at Winthrop University.
If you are a citizen of the United States, please sign this form, attach one of the listed documents
and mail the form and attachment to:

Valid Documents:
o South Carolina Driver’s License if you were first licensed after January 1, 2002.
(Winthrop may verify the license with the South Carolina Department of Motor Vehicles.)
o Current and valid driver’s license from any US state EXCEPT California, Colorado,
Connecticut, Delaware, Hawaii, Illinois, Maryland, New Mexico, Nevada, Utah, Vermont,
and Washington.
o Certified Birth Certificate indicating you were born in the United States or a territory of
the United States, with your given name and surname, date and place of birth, date birth
record was filed and the seal or other certification of the official custodian of the record.
This certificate will be retained as a part of your student record.
You may also verify your status using one of the following documents:
o Current US Passport
o Certificate of Naturalization (USCIS Form N-550 or N-570)
o US government issued Consular Report of Birth Abroad
o Certificate of Citizenship (N-560 or N-561)
o Unexpired US Active Duty/Retiree/Reservist Military ID card (DOD DD-2)
o Social Security Card
Copies are acceptable but Winthrop retains the right to ask for the original if needed.
