Exposure B: a structure located at least 1,500 ft from the shoreline of open water
Exposure C: a structure located at least 600 ft from the shoreline of open water & less than 1,500 ft
Exposure D: a structure located within 600 ft of the shoreline of open water

Window/Door Replacement and/or Impact Shutter Permit Form.

1. This diagram will be used to depict the location of the existing windows/doors and to identify which window/door units are being replaced.
2. Draw a plan view outline of the structure. The structure shape/configuration must match, scale is not required.
3. The front of the structure faces the bottom of the page.
4. Mark the location of the units to be replaced with an “X” and identify the units with a location number that corresponds with your
Florida Product Approval Schedule location number.
5. Identify the unit size (width x height) and the unit type (SH = Single Hung, DH = Double Hung, SGD = Sliding Glass Door, etc.)
6. Provide a separate Layout Diagram for each Floor Level/Story where window/door units are being replaced or shutters are being installed.
