PLEASE READ ALL INSTRUCTIONS CAREFULLY!
The Fisher College of Science and Mathematics (FCSM) Scholarship Committee is accepting applications for the above
tuition four-year scholarships.
Only new, incoming, full-time freshmen students from Baltimore Polytechnic Institute, who are planning to major in FCSM, are
eligible to apply. Selection of awardees will be made on the basis of merit without regard to race, religion, or gender. Awards will
cover tuition at the in-state rate. Merit will be determined by the student’s class rank, GPA, SAT/ACT scores, and the number,
quality, and performance in science and mathematics courses. Awards are renewable up to four years if the awardees make
satisfactory progress in their FCSM major and maintain an overall minimum GPA of 3.0 . If an awardee’s GPA falls below the
stated minimum in a given year, they will have one semester to rectify the situation. To apply, complete this application in its
entirety.

SAT/ACT Scores may be downloaded from College Board or included on transcript but must be included with this form.
SECTION IV – Two Signed and Sealed Letters of Support on School Letterhead from High School Science or
Mathematics Teachers Only.
Each science or mathematics teacher must compose and sign a letter of support and complete the teacher recommendation form
to be included with this form in a sealed envelope signed by the teacher.

All student recipients are expected to write a thank you note to the donor or donor representative as appropriate. The
recipient will be contacted by the Office of Development with more information on the acknowledgment process soon
after notification of selection.
