Release of Information
In accepting the James and Catherine Buck Charitable Trust Scholarship, I understand the provision
of my grade point average is a specific requirement of the trust. In consideration of receiving this
scholarship support, I authorize Wichita State University to release my grade point average to the
Bank of America, NA as trustee for the James and Catherine Buck Charitable Trust and further
waive my rights under the Family Educational Rights and Privacy Act of 1974 for this purpose.
