from your application with your financial documents. The law says we have the right to ask you for this information before awarding Federal
aid. Complete this verification form and submit it to the MSU Northern Financial Aid Office as soon as possible, so your financial aid won’t
be delayed.
High School Completion Status: (check one of the boxes below)
High School Graduates:
o Must provide a copy of final high school transcript which includes the date the student completed high school, or
unavailable a high school diploma
GED recipients:
o Must provide a copy of GED certification or transcript
Recipient of a state certificate:
o Must provide a copy of the certificate the student received after passing a state authorized examination which the
state recognizes as the equivalent of a high school diploma
Student who has successfully completed at least a two-year program:
o Must provide a copy of the academic transcript showing the student has completed at least a two-year program
acceptable for full credit towards a bachelor’s degree.
Home Schooled students:
o Must provide a transcript or the equivalent signed by the student’s parent or guardian that lists the secondary school
courses completed by the student and documents the successful completion of a secondary school education; or a
secondary school completion credential for home schooling (other than a high school diploma or its recognized
equivalent) acceptable under state law.
Student who is seeking enrollment in an associate’s degree program or higher who did not complete high school but excelled
academically in high school:
o Must provide documentation from the high school that the student excelled academically; and documentation from
the postsecondary institution that the student has met its formal, written policies for admitting such students.
