Not required - Test scores are not required if overall bachelor's degree in nursing GPA is 3.0 or higher from a
regionally accredited institution
Required - Test scores are required if overall bachelor's degree in nursing GPA is between 2.75 and 2.99 from a
regionally accredited institution
NOTE: Applicants with an overall bachelor's degree in nursing GPA of 2.749 or lower will not be considered for
admission
I understand the following:
1) The waiver request form must be uploaded into the self-service portal post submission.

2) By signing this document, I am acknowledging that I have an overall bachelor’s degree in nursing GPA of a 3.0 or
higher from a regionally accredited institution and will not be required to submit GRE or MAT test scores.
I understand that if I do not meet the specified GPA criteria for the waiver and I submit my application, I will be
ineligible for consideration.
Applicant Action: Upload a copy of the approved waiver form into the online application post submission. It is the
responsibility of the applicant to ensure that the requirements checklist within the online application for admission is
complete prior to the published application deadline.
