Total fee
It is hereby agreed by the undersigned, as owner, owner’s agent or contractor and the Village of Caledonia that for and in consideration of, the
premises and the permit for execution of installation of fire protection equipment, or any other items listed above, is to be issued and granted by
the Fire Department. That the work thereon will be done in accordance with the description herein set forth in this statement. It is further agreed
to alter or install in strict compliance with the Village of Caledonia Fire Prevention Code and to obey any and all lawful orders of the Village of
Caledonia Fire Department, the Statutes of the State of Wisconsin and rules and regulations issued by the Department of Commerce under the
authority of the State Statutes. Note:
The Village of Caledonia and/or the Village of Caledonia reserve(s) the right to refuse to issue, modify
and/or revoke any permit for cause including but not limited to false and/or inaccurate information being provided.
System inspections and/or witnessing of tests are mandatory. Witnessing of ALL tests and inspections MUST
BE scheduled a minimum of 72 hours advance. CALL the Village of Caledonia Fire Department (262) 835-2050
for scheduling.
