Yellow Ribbon Application Form
STUDENT INSTRUCTIONS: COMPLETE EVERY SEMESTER
Yellow Ribbon Program
The Yellow Ribbon Program (YRP) is a provision of the Post-9/11 Veterans Educational
Assistance Act of 2008. This program allows institutions of higher learning in the United States
to voluntarily enter into an agreement with the VA to partially or fully fund tuition expenses that
exceed the current thresholds under the Post 9/11 GI Bill®.
Mohave Community College and the VA will make equal contributions toward the program on
behalf of the individual in the form of a grant or scholarship, with the total to not exceed $500.00
during the 2017-2018 academic school year for 20 eligible students.
Who should apply?
o Veterans that reside out-of-state
o Veterans who are not eligible for Western Undergraduate Exchange (WUE)
Yellow Ribbon Eligibility Requirements:
o Only veterans and their spouses using transferred entitlements, eligible at the 100 percent
(100%) benefit level (which is based on service requirements) may receive funding
o Transfer of Entitlement (ToE) children are eligible for YRP if the sponsor is eligible at
the 100 percent (100%) benefit level, regardless of the sponsor’s active duty status
Documents needed prior to approval:
o Current Certificate of Eligibility (COE)
o Completed Yellow Ribbon Program Application
o Certificate of Enrollment Form
o Promissory Note
STATEMENT OF ACKNOWLEDGEMENT
o I have a degree declaration on file.
o I have completed the enrollment process.
o I have submitted my application with the Department of Veterans Affairs (VA) to begin
receiving my education benefits under Chapter 33 Post 9/11 GI Bill®.
o I have provided a recent copy of my Certificate of Eligibility (COE).
