Vermont National Guard Members: You must be a “Veteran” as defined in 38 U.S.C. to qualify.
Registration
If this is a new or transfer registration, form VD-119, Vermont Registration, Tax & Title Application must also be submitted
along with the required fees. If renewing registration, include renewal notice. **Disabled plate requires additional verification
AND completed DMV form VS-113 (unless you already have a disabled plate or parking placard).
The Commissioner of Motor Vehicles shall, upon proper application, issue special plates to Vermont veterans, as defined in
38
U.S.C. § 101(2), and to members of the U.S. Armed Forces, as defined in 38 U.S.C. § 101(10). The plates shall be reissued only
to the original holder of the plates or the surviving spouse.
The Office of Veterans Affairs will return the completed application with Veteran certification back to you, if your registration
expires soon. Otherwise the Office of Veterans Affairs will forward the application to DMV. Once you have received the returned
application bring this application to a DMV office or mail the application to the DMV. Be sure to also have your renewal notice
or new registration application.
License & Non-Driver Identification cards
This application must be submitted with appropriate license application and fees. If a renewal, include license renewal notice. If not a renewal
(or if renewal notice is not available) use form VL-21 for Driver’s License or form VL-31 for CDL.
If a veteran, as defined in 38 U.S.C. § 101(2), requests a veteran designation and the Office of Veterans Affairs
confirms his or her status as
an honorably discharged veteran, the license or Non-Driver ID certificate shall include the term "veteran" on its face.
The Office of Veterans Affairs will return the completed application with Veteran certification back to you, if your license expires soon.
Otherwise the Office of Veterans Affairs will forward the application to DMV. Once you have received the returned application bring this
application to a DMV office or mail the application to the DMV. Be sure to also have your renewal notice or new license application.
38 U.S.C. § 101
(2) The term “veteran” means a person who served in the active military, naval, or air service, and who was discharged or released
therefrom under conditions other than dishonorable.
(3) The term “surviving spouse” means…a person…who was the spouse of a veteran at the time of the veteran’s death, and who
lived with the veteran continuously from the date of marriage to the date of the veteran’s death…and who has not remarried….
NOTE: Surviving Spouse may only be reissued an existing plate.

DD-214 (which includes “character of service”) to:
