Verification information
What is verification and why was I selected?
Verification is the process by which certain required information on the FAFSA is reviewed for accuracy
and completeness.
Students are selected for verification for one of the following reasons:
The Department of Education Central Processing System found inconsistencies and/or potential
errors in the student’s FAFSA
Random selection
School selection
Steps to complete verification:
Complete all required sections of this worksheet, sign it, and send the completed worksheet along
with any supporting documentation to the Office of Student Financial Aid.
The Office of Student Financial Aid will notify you via your Edison email account if there is additional
documentation needed or any other questions to be answered.
The Office of Student Financial Aid cannot process your financial aid package and
determine your aid eligibility until verification has been completed, so it is imperative that
you provide the required documents as soon as possible.

*** ONLY Complete this section if the STUDENT AND/OR SPOUSE did file or
will file a 2018 Federal Tax Return.
The best way to verify income is by using the IRS Data Retrieval Tool (IRS DRT) that is part of FAFSA
on the Web at FAFSA.gov. In most cases, no further documentation is needed to verify 2018 income
information that was transferred into the student’s FAFSA using the IRS DRT if that information was not
changed by the FAFSA filer.
Note: If the student and spouse filed separate 2018 IRS income tax returns, the IRS DRT cannot
be used and signed copies of the 2018 IRS 1040 Federal Tax Return must be provided for each
individual.
Check all that apply:
The Student (and/or spouse) has used the IRS DRT in FAFSA on the Web to transfer 2018 IRS
income tax return information into the student’s FAFSA.
The Student (and/or spouse) was unable or chose not to use the IRS DRT in FAFSA on the Web,
and instead must provide the school with a signed copy of their 2018 IRS 1040 that was submitted
to the IRS, including Schedules 1, 2, and 3 if applicable.
The student and spouse filed separate IRS income tax returns for 2018 and have provided signed
copies of 2018 IRS Federal Tax Returns for each individual.
(If you do not have a copy of your 2018 IRS 1040, a 2018 IRS Tax Return Transcript may be obtained
within 10 business days through:
Automated Telephone Request Call the IRS at 1-800-908-9946.
On-line Request - Go to www.IRS.gov, under the Tools heading on the IRS homepage, click “Get
a Tax Transcript.” Click “Get Transcript by MAIL” or “Get Transcript ONLINE.” Make sure to
request the “IRS Tax Return Transcript” and NOT the “IRS Tax Account Transcript.” To use the
Get Transcript ONLINE tool to get immediate tax transcript access, the user must have access to
valid email address, a text-enabled mobile phone, and specific financial account numbers such as
a credit card number.
Paper Request Form - IRS Form 4506T-EZ or IRS Form 4506-T.)

non-filing can be obtained from the IRS by the same methods used to obtain a tax return transcript.
