Your application for federal financial aid was selected for review because of conflicting information with regard to your high
school completion status. The law says we must resolve this conflict before awarding financial aid. Based on the information
you provide below, corrections may need to be made and your information will be reprocessed.
You, the student, must complete and sign this form as soon as possible. Contact the Financial Aid Office if you need

A. HIGH SCHOOL COMPLETION STATUS
You, the student, must provide the Financial Aid Office with one of the following documents that indicate your high school
completion status at the start of the academic year.
A copy of your final official high school transcript that shows the date when the diploma was awarded.
A copy of your General Education Development (GED) certificate or GED transcript.
If homeschooled, a copy of a state secondary school completion credential, OR, a transcript or the
equivalent, signed by your
parent or guardian, that lists the secondary school courses you completed
and documents the successful completion of a secondary school education in a homescho
ol setting.
A copy of your high school diploma.
An academic transcript that confirms you successfully completed at least a two-year program that is
acceptable for full credit toward a bachelor’s degree.
IMPORTANT NOTE: If you are not able to provide any of the documents listed above, please contact
the Financial Aid Office as soon as possible as your eligibility for Federal student aid may be affected.
