I acknowledge that it is required that I earn a minimum grade of “B” in all graduate-level coursework. As applicable, I must still apply and be
admitted to the graduate program as a 4+1 student before I am considered a graduate student. I understand that no more than 12 credit hours
of graduate coursework taken as an undergraduate can be counted towards a graduate degree. Only 6 of these 12 credits can also be used
towards the undergraduate degree. Permission to enroll in graduate coursework or the 4+1 program is not a guarantee of graduate program
admission.Students retain their undergraduate student status until they graduate with their Bachelor’s degree, apply for, and are admitted to a
graduate degree program. I certify that all answers and statements are true and complete to the best of my knowledge.
