1 Come to the Copyright Oce, Madison Building, 101 Indepen-
dence Avenue, Washington, DC, where copies of the original
registrations and catalog entries are available to the public. (Call
the Copyright Oce Public Information Oce at (202) 707-3000
or visit the Copyright Oce website for information about public
hours.)
2 Search the Catalog of Copyright Entries at a public library.
3 Request an ocial search by the Copyright Oce. (See Circular 22
about requesting a search and the accompanying fees.)

Section 1
·
Title Information
In line 1a give the title of the work upon which eligibility for regis-
tration is based. If the work is a periodical, give the volume, number,
and issue date. If copyright is claimed in a contribution to a peri-
odical or other collective work, give the title of the contribution in
line 1b.
Section 2
·
Author Information
In line 2a give the name of one author upon whom eligibility for
registration is based. In line 2b specify the author's citizenship
and
domicile at the time the work was rst published. If the work was
created as a joint work by multiple authors, only one author should
be named.
note:
For information about international copyright rela-
tions of the United States, see Circular 38.
Section 3

Publication Information
Give the complete date (month/ day / year) and the nation of rst
publication. If the work was published simultaneously in the United
States and a foreign country, state U.S. as the nation of publication.
Section 4
·
Manufacture Information
Complete this section
only
if the work consists predominantly of
nondramatic literary material, including periodicals and two-di-
mensional prints and labels, in the English language. If all of the
processes associated with the manufacture of copies of the work
were performed within the limits of the United States, check that
option. Otherwise, specify the nation where the work was manufac-
tured and the processes used in its manufacture. For example, you
might use terms such as “typeset, platemaking, lithographic pro-
cess, photoengraving process, printing and binding” to describe the
process(es), as applicable.
Section 5

Deposit Information
A deposit copy is required
only
if this is the rst registration of the
work, or of the collective work in which the contribution was rst
published. The deposit requirement is one complete copy of the
work as rst published.
Check
option 1
if you are sending a complete copy of the rst
published edition. If not, check
option 2
and describe the alternative,
acceptable material deposited, such as a complete photocopy of the
published edition or a reprint or reissue. If you check option 2, you
must include a photocopy of the title pages, the credits, or the album
cover of the work as rst published to show proof that the work was
rst published with an acceptable copyright notice. If you are unable
to send a complete photocopy or reprint or reissue, the Copyright
Oce may accept the following identifying material:
Printed literary, dramatic, or visual arts work ·
A photocopy of the
following pages from the published edition: (1) title pages showing
the copyright notice (and manufacturing imprint for literary works,
prints, and labels), (2) table of contents pages, and (3) rst and last
25 pages containing a representational amount of the authorship
claimed.
Musical work ·
A photocopy of the following pages from the
published edition: (1) title pages showing the copyright notice, (2)
list of contents pages, if any, and (3) at least one-third of the musical
content, representing the authorship claimed. If the copyright notice
is not on the title page(s), include the page containing it and explain
its position in relation to the work as a whole.

Contribution to a periodical or other collective work ·
A photocopy
of the following pages from the published edition: (1) masthead (for
periodical), (2) title page(s) of the larger work showing the copyright

work of authorship claimed.
Sound recording ·
A compact disc or cassette containing the
entire sound recording as rst published, and (1) photocopy of both

Read these instructions before completing the application.
Use this form for
1 Works rst published
from January 1, 1964, through December 31, 1977,
but not registered
during the original term of copyright. This includes published sound recordings xed on or
after February 15, 1972.
note:
Sound recordings xed prior to that date are not eligible for federal
statutory protection with the exception of certain foreign sound recordings which may be eligible
under the Uruguay Round Agreements Act. See Circular 38. Such works cannot be registered with a
Form RE/Addendum.
2 Contributions, including prints and labels, by U.S. authors which were rst published abroad
in English-language periodicals or other collective works.
