Congratulations on completing the requirements to be recognized as a Unit of Excellence Award recipient. The Unit of
Excellence Award is a patch (pictured in the upper right corner of this form), which can be worn proudly in the temporary
patch location of your uniform.
Requirements:
Scout: serve as the OA Troop or Team Representative for at least 6 months and lead the unit to earning the Unit of
Excellence Award.
Adviser: advise at least two OA Troop/Team Representatives towards earning the Unit of Excellence Award.

requirements. Then mail this form and a $3.50 patch fee to the address listed below. Please allow 30 days for
processing.
