2018-2019 Unusual Enrollment History (UEH)
The U.S. Department of Education established new requirements to prevent fraud and abuse in the federal programs by
identifying students with an unusual enrollment history.
Many students with unusual enrollment history may have legitimate reasons for their enrollment history at multiple
institutions. However, such enrollment patterns now require an institutional review to determine whether valid,
documentable, reasons exist for the unusual enrollment. This must be satisfactorily resolved by the institution before
the student can receive financial aid. Students may also be selected for this process if they received a Federal Pell Grant
and/or Federal Direct Loan(s) during the last four consecutive years but did not successfully complete the number of
hours for which the Federal Pell Grant and/or Federal Direct Loan(s) was provided. Further information may be
requested after initial documentation received has been reviewed.
In order to resolve this issue, you must:
• Complete this form in its entirety; and
• Provide an official or unofficial copy of all college transcripts from Fall 2014 through Summer 2018 to JU for
official evaluation (even if previously provided for admissions purposes); and
• Go online to the National Student Loan Data System (NSLDS) at www.nslds.ed.gov/nslds _SA/
and print a copy of
the institutions you have attended while receiving aid. Attach the printout to this form.
