This form should be used when a college or school wants to propose a new degree offering. It will typically
be accompanied by a proposal for a new or revised major/minor/program curriculum and potentially a
proposal(s) for a new or revised course(s). When the form is complete, it should be sent to the appropriate
curriculum committee to initiate the process of faculty review. The Faculty Organization Plan requires that
curriculum committees review curricular proposals in terms of their education quality, content and
consistency with the mission of the University.
