If you are a veteran, please submit this form and a copy of your Certificate of Release or Discharge from Active Duty (DD Form 214) in the
Supplemental Information section of your application. If you are an active duty service member, please submit this form and a copy of your
Status Report Pursuant to Servicemembers Civil Relief Act (as downloaded from https://scra.dmdc.osd.mil/welcome.xhtml) in the Supplemental
Information section of your application.
The request takes 2 to 3 business days to process. Please wait for an e-mail stating that your request has been approved before submitting your
While your DD Form 214 or Status Report may be provided after this form has been submitted, please note that your application fee waiver will
not be granted until both this form and your DD Form 214 or Status Report have been received. Once an application fee has been paid, it may
not be refunded. If you submit this form and a copy of your DD Form 214 or Status Report after an application fee has been paid, the
documentation will still be used for an admission deposit waiver if you are admitted to a graduate program.
