PURPOSE: "Inheritance" is the practice of passing on vehicles upon the death of an individual. If the deceased left
no will (intestate) and there is not a surviving spouse, this form must be completed to enable all heirs to mutually
relinquish their ownership of a vehicle to a certain party.
INSTRUCTIONS: Complete this form in its entirety and submit it to your local county clerk's office.
