Please complete and submit this form to request an exemption from the requirement to submit a survey plan dataset
for a given plan. Please note that Practice Bulletin No. 3 requires that datasets be submitted for plans filed after
June 15

, 2016 with an electronic checklist date of May 23

, 2016 or later and for plans filed after September 1

2016 regardless of the electronic checklist date. If based on these dates you are not required to submit a dataset
then no formal exemption approval is required.
