Supervised Exam Procedures
(For Use With Independent Exam Supervisors & Consortium of College Testing Centers)
Each CyberCampus (entirely online) course is required to have at least one supervised exam. To take this exam
you must submit the name of an acceptable person who can supervise your exam(s). Some courses will have a
web-based supervised exam. For web-based exams students will need access to a computer for up to 3 hours
with reliable Internet access conforming to system requirements consistent with a CyberCampus course.

Student Guidelines and Responsibilities
Students are responsible for securing the services of a suitable Exam Supervisor according to the requirements
listed below. This action must be completed no later than two weeks after the first day of class to ensure
adequate time for the Exam Supervisor Approval process. The Exam Supervisor must be approved before the
midterm and/or final examination is sent out.
If the Exam Supervisor charges a fee, the student must make payment arrangements.
If you do not select a supervisor, you will not be able to take your exam. If you have questions about this

Your Supervisor will be given instructions by your professor and will be responsible for monitoring you while
you take your course exam(s).
Exam Supervisor Guidelines and Responsibilities
1. The Exam Supervisor will be responsible for the following:
a. Receiving the exam sent by CyberCampus, or receiving a secure password for web-based supervised exams.
b. Supervising the exam for the student while adhering to the exam instructions provided. Exam instructions
will include:
-length of exam (not to exceed 3 hours)
-materials students may use (open/closed book etc.)
-what date the exam must be taken by
-the address to mail the exam*
-instructor's contact information
-affidavit of student identity
c. Mailing the exam back to the address on the exam instructions by mail.*
* Web-based supervised exams will not require mailing
d. For web-based supervised exams, a secure password will be sent to the exam supervisor from CyberCampus. This
password will open the online supervised exam.

2. The criteria established for Exam Supervisor approvals are:
a. Knowledge of and experience in the administration of examinations.
b. A professional reputation which would be severely tarnished if improprieties were discovered.
Based on our accreditation, acceptable Exam Supervisors are:
a. a work supervisor (must be someone in a supervisory capacity over the student)
b. a faculty member or administrator of an accredited institution of higher education
c. a school teacher
d. a full-time school or local librarian
e. an officer with a higher rank than the student, if either party is in the military
f. a local college or testing center
g. any member of the Consortium of College Testing Centers. To find a member in your area, go to

Please note: a proctoring fee may apply for certain testing centers.
Based on our accreditation, non-acceptable Exam Supervisors are:
a. relatives
b. spouses
c. friends
d. neighbors
e. co-workers
Approval Process:
Once you submit the attached form, you will be notified if your exam supervisor is approved or disapproved.
Please Note: complete one form per term for all courses during the term.
If approved, you will be notified by e-mail of approval and your exam will be sent* to your Exam Supervisor
at the requested address on the following form. The student is responsible for scheduling a time and date to take
the exam with the exam supervisor. *password sent directly to exam supervisors for web-based exams.
If disapproved, you will be sent e-mail with a reason for non-acceptance of your original choice and you will
have to download a second supervisor form, which can be found by going to
http://www.ggu.edu/cybercampus and
selecting the ‘Supervised Exam’ button.
