On your 2018/2019 Free Application for Federal Student Aid (FAFSA), you indicated that at some point
after you turned age 13, 1) neither of your biological or adoptive parents were living; 2) you were in
foster care (even though you may not be in foster care now); or 3) you were a legal ward/dependent of
the court (even though you may not be a ward/dependent of the court now.) Please note that for the
purposes of awarding federal financial aid, a student who is/was incarcerated is NOT considered a
ward of the court.
Your response to this FAFSA question must be verified before processing of your aid eligibility can
continue. Therefore, please submit documentation verifying at least one of the above circumstances.
This can be a photocopy of a legal document.
The required document or letter should be attached to this form, which must be signed and dated
