Street, San Francisco, CA 94105-2968. To change your legal names, submit the “Student Information
Change Request” form with supporting documentation.

*Students may declare both first and last names; if a chosen first or last name is not declared, the
student’s corresponding legal name will be used.
Please note the following policies and disclaimers:
• Golden Gate University recognizes that some students prefer to be known by chosen or
preferred names rather than their legal names. The university strives to use and display
chosen names wherever and whenever possible in its student records, learning
management, and communications information systems. To that end, once declared,
chosen names will appear in GGU4YOU, eLearning and other information systems where
students’ instructors or classmates can see their names. In addition, student’s preferred
pronouns will appear on instructors’ GGU4YOU class rosters.
• The first letter of each declared name will be capitalized when entered into the
university’s student records system. If a chosen middle name is desired, it must be
entered together with the chosen first name (e.g. Mary Beth).
• The use of chosen names for purposes of misrepresentation or fraud will be considered
a violation of the student code of conduct.
• After submission of this form to the Registrar’s Office, students may obtain student ID
cards showing their chosen names from the GGU Hub, free of charge.
• Chosen names will not be used where legal names are required by law or institutional
policy, such as: financial aid records, official transcripts, diplomas, payroll records, and
federal immigration records.
• To change their chosen names or to have them removed from their records, students
must resubmit this form.
• The university is engaged in several projects designed to propagate chosen names to its
various student information systems. Consequently, some systems may not be able to
accommodate a student’s chosen name until a future date.
