This Student Appeal for Policy/Procedural Exception Form
is used to request consideration for an exception to established HawaiI CC policy and procedure in the event of
extenuating circumstances that are beyond the student’s control (i.e. serious illness or injury, a death in the family, involuntary job transfers, military service, and
institutional error. A request for an exception to another campus' policy or procedure must be made to that campus and cannot be approved or disapproved on behalf of
another campus.

When Submitting an Appeal:
Forms must be completely filled out and signed with supporting documentation.
The Admissions & Records Office reserves the right to verify all information, including verifying with
physicians and instructors, and may review attendance, course performance, and all other available
academic progress.
Failure to respond to requests for documentation or further information within 14 business days of
notice will result in an automatic denial of the appeal.
If you are unable to add or drop courses through STAR GPS, attach a completed Add/Drop Form.
Registration changes could affect future eligibility for financial aid and may result in a financial
obligation. If you are receiving financial aid, you should discuss your request with your Financial Aid
Advisor prior to submitting this form. Registration after the deadline requires the instructor(s) signature
on the Add/Drop form. Instructors must also input registration overrides(capacity, prerequisite, etc.)
into BANNER.
If you are requesting to lift a financial obligation, please contact our Business Office and attach a copy of
your payment plan agreement. A payment plan is required, please attach copy and contact the Business
Office at (808) 934-2740 to set up a payment plan. Students are responsible for payment of all tuition
and fees that are still assessed with their account after the appeals process.
Appeal Process:
The Registrar’s Office reviews appeals on a time permissive basis. Normal processing time is from
3-4 weeks; however, depending on the complexity of the appeal, receipt of supporting
documentation, and/or time to communicate with other involved parties, processing time may
extend beyond that time frame.
All appeals are reviewed on case-by-case basis and all appeal decisions are final. Students will be
contacted via their UH email as indicated on this form with requests for additional information and
appeal results.
