Salt Lake Teens Write
Applicant's Name
The applicant is applying for admission to Salt Lake Teens Write, a mentoring program co-
sponsored by the Salt Lake City Library and the SLCC Community Writing Center that matches
local teens who are entering the 11th Grade with adult mentors who use writing in their daily and
professional lives. These mentoring teams will be meeting one-to-one for at least one hour a week
from during the school year. The teams will also meet on a quarterly basis with the other
participants in the Salt Lake Teens Write program for a variety of writing workshops and public
readings.
As you complete this recommendation form, please understand the the program is currently
conducted only in English.
